If your child wears an ankle-foot orthosis (AFO), you already know the daily routine: wrestling a rigid brace into a shoe that was never built to open wide enough, fighting the heel, loosening the laces all the way down, and hoping the fit holds for the day. The right shoe turns that battle into a few seconds. This guide explains what actually makes a kids' shoe work over an AFO, and which BILLY styles are engineered specifically for braces.
An AFO adds rigid volume around the heel, ankle, and along the foot. A standard shoe opens only at the throat near the laces, so the brace has to be forced down past a fixed heel counter and collar. That creates three problems: the shoe won't open far enough to get the braced foot in, there isn't enough depth or width to hold the brace plus the foot comfortably, and the closure can't be adjusted enough to secure both. The result is rubbing, pressure points, and a daily struggle.
Four features matter, and a shoe really needs all four:
BILLY Footwear was built around the first point - the patented zipper opens the shoe almost completely, folding the upper back so a braced foot sets straight in with no forcing. Two of BILLY's lines take it further and are engineered specifically for AFOs.
The BILLY Goat was developed in collaboration with Boston Orthotics & Prosthetics and is CPMA-certified. It's purpose-built for braces: two removable 3mm insoles plus a wedge insert let you dial in the internal depth around an AFO, a wrap-around zipper opens it wide for easy on-and-off, and a double-layer toe cap stands up to the wear that bracing and toe-drag put on a shoe. For families who need maximum room and adjustability, the Goat is the place to start. There's also a dual-zipper Goat with an opening on each side for the easiest possible entry.
The BILLY Classic D|R II sits on a deeper last with extra room for orthotics and braces, and it's CPMA-certified. It adds an Achilles notch that clears the hardware on hinged AFOs, two removable 3mm insoles for depth adjustment, and reinforced construction made to last. It comes in Medium, Wide, and Extra-Wide (MDR, WDR, XDR), so you can match the width to your child's foot and brace.
Because the zipper travels down and around the shoe, the entire upper folds open and flat. You set the braced foot in from above, fold the upper back over, and zip - no forcing past a heel counter, no fully un-lacing. Many kids can do it themselves, which is often the first time a child in AFOs can put their own shoes on independently. That independence matters as much as the fit.
A braced foot needs more room than a bare one, so children in AFOs often size up from their regular shoe size and benefit from a wider width. The most reliable approach: bring your child's AFO to the fitting, remove one insole layer to create depth, and check that there's room at the toe with the brace fully seated. Your child's orthotist or occupational therapist knows the brace best - when in doubt, fit the shoe with their guidance. If you're measuring at home, our team is happy to help you choose a size and width.
